Well, those papers are exceedingly important and she certainly needs to make copies, and Hang onto them when she gets the NEW copies she needs. She needs to call that child support division of Yakima county and ask them to look up the records. Then she will need notarized copies of those. . There is a special form that can be used for applying for a passport for a child if it is totally impossible to get the signature of the other parent...see site below. However, she should be able to get that form. She also needs a signed, notarized letter of permission from the father to take the children out of the country...unless she has documentation that she has sole custody....so she needs that document.

Proof of custody in Oregon? -
My mother is trying to obtain a passport for my two younger brothers and is being asked for proof that she has sole custody of the children. How can she obtain this proof?Their father has signed a paper in 2002 that said he gave the full and sole custody to her while he was in prison but she can not find the papers...we don t know where we can ask for a copy or something as it was done in Washington and we now live in Oregon. Is there any way to prove that she does indeed have the sole custody as it must be in record SOMEWHERE?It was done through the child support division in Yakima County, Washington in 2002Please Help!
