The receipt must state applicants�� names, amount paid (1050 SEK per applicant) and bankgiro number. Make sure that your bank issues something verifiable with the above information. Internet receipts will not be accepted.? electronic DS-156 application form filled online and printed out, hand-written applications are not accepted; document must be signed? downloaded and completed DS-157 application form? a self addressed stamped envelope (stamps worth 62 SEK must be put on envelope) for the return of your passport. The passport will be sent back with registered mail. You need to show ID to be able to obtain the parcel from the postal services. If the passport is your only means of ID you need to address the envelope to someone else.? a police record in original, copies of court records and of any prison records should you have been arrested or convicted for any offense or crime. ? a current bank statement or other evidence to show that you have sufficient funds to cover the costs of the trip. If someone else will be financing it a bank statement and a letter of consent from this person.? an employment letter stating the type of contract, how long you have been employed for and for how long you have been granted vacation or leave.
